Submission and verification may take up to 48-72 hours after sharing your application number.
No applications are processed over weekends or public holidays.

Form 1A Information & Important Notes:

Verify all Form 1A requirements (signature color, eligible doctors, procedures) independently. Information provided is for guidance, but accuracy is your responsibility.
Certificate issuance occurs only after payment.

Delivery options: Digital (WhatsApp/Email) or Physical (Post).

Steps to Obtain Form 1A:

Fill out the Form 1A.
Take the online ear and eye test.
Get Form 1A attested by a doctor within the required timeframe.

RTO & Form 1A Acceptance:

Form 1A acceptance is determined by your local RTO.
Confirm their specific requirements before using our service.
We cannot provide green pen signatures.
Green ink signatures are only valid when provided by government hospital doctors, physically, on hospital/PHC premises.
We cannot alter your address, photo, or other Form 1A information.
The doctor assesses your medical condition based on the Parivahan site-generated form using your application number.

Signed Note from RMP (Registered Medical Practitioner):

The signed note provided after your tests is only valid for online submissions.
For offline submissions:
Photo must be attested with a physical stamp and signature from the RMP.
Digital certificates are electronically generated.
Handwritten certificates are manually filled and sealed by the doctor.

We are not responsible for medical data updates on government portals.
We work with certified partners. Doctor personal/qualification details are not shared to prevent misuse.

I agree to a Rs 200 convenience fee for non-issued certificates with refundable document formats. Non-refundable formats are not refundable. Refunds are processed online within 8-10 business days. I agree to the Refund and Cancellation Policy and other terms. This online consultation doesn't replace an in-person doctor's appointment. I've read and agreed to the terms of service, including using my application number to access Form 1A from the Parivahan site as per their privacy policy.

When you submit documents to the RTO online on the Parivahan site, they may request certain cases to visit the RTO for physical document verification. You will need to submit a physically attested copy of your Form 1A for your appointment.
